Students seeking transcripts from closed schools that were previously registered with the State Board of Career Colleges and Schools should review the closed school list below to determine who to contact about their transcript. 

If the list below indicates that your transcript may be kept by the State Board of Career Colleges and Schools, you may obtain a copy of their records by submitting the following form to the Board:

Please be advised that any request to access student records or transcripts may require the Board to access and review confidential personal information in accordance with Ohio Revised Code Chapter 1347.

Students who need a copy of their transcript and whose records cannot be located may request that the State Board of Career Colleges and Schools recreate a copy of their transcript and/or diploma where sufficient records can be obtained (from your personal records) to make the recreation possible. This may include grade cards, transcripts, diplomas or other documents concerning your education that you have in your possession. There is a $15 charge for each transcript or diploma recreation. Please contact the Board to determine if this is a viable option. Once the Board has received the necessary fee, records will normally be sent within 15 days.
